Job summary

VITAS Healthcare is seeking a Telecare RN in Phoenix, AZ to join the interdisciplinary team after normal business hours. The RN identifies physical, psychological, social, and spiritual needs and initiates interventions using the nursing process and Telecare Protocols.

Responsibilities include coordinating after-hours service delivery, documenting care, collaborating with physicians and staff, and pursuing hospice nursing certification when eligible.

Qualifications

• Minimum 2 years’ acute-care hospital nursing in medical-surgical, oncology, home health, or emergency.
• 1 year of customer service/call center experience preferred.
• Must live within 75 mile radius from Phoenix, AZ.
• Eligible for licensure in other states VITAS Triage services are located.

Responsibilities

• Provide clinical assessment and intervention using nursing process and Telecare protocols.
• Coordinate after-hours service delivery including education and coordinating visits.
• Collaborate with attending physicians, LTC staff, case managers and external vendors.
• Document patient/family interactions in the medical record with plan of care.
• Review entries by LPN/LVN and Coordinators for compliance with standards.
• Attend in-services, meetings, and educational conferences.
• Pursue specialty hospice nursing certification when qualified.
• Instruct on medications, home medical equipment, and supplies.
• Support Telecare staff to improve patient outcomes.
• Promote customer-service oriented care and disaster planning adherence.
