Job summary

Qualifications

• Bachelor’s Degree in Human Services required.
• Master of Social Work (MSW) preferred.
• Bilingual Spanish preferred.
• Minimum 2+ years of experience counseling individuals with substance use disorders and/or mental illness.

Responsibilities

• Provides case management to Health Home eligible adults with severe mental illness and SUD service history.
• Provides individual counseling and facilitates caseload groups and other assigned groups.
• Supports community service linkages and referrals to social supports.
• Provides transitional care and navigation of complex health and social services.
• Provides individual and family support and advocates for needed services.
• Performs on-site crisis intervention and skills teaching when needed.
• Monitors treatment plans to ensure needs and recovery goals are addressed.
• Monitors follow-up with clients, families, collaterals, and service providers.

Education

Bachelor’s Degree in Human Services
