Job summary
The Community Supports Network is seeking a Care Coordinator to lead our team in delivering quality services that empower individuals to learn, grow, and live their best lives within the community. This salaried role may require evening and weekend availability.
Responsibilities include coordinating medications, accompanying clients to doctor appointments, documenting care, and coaching Direct Support Professionals. A strong focus on safety and compliance is essential.
Qualifications
- 1 year experience in field of intellectual disabilities and/or behavioral services.
- High school diploma or GED.
- Valid driver’s license and current auto insurance.
- Reliable vehicle to transport during shift.
Responsibilities
- Assist with medication management
- Support individuals to practice and maintain good hygiene
- Follow health & safety protocols
- Engage and coach Direct Support Professionals
- Schedule ALL doctor Appointments
- Accompany supported individuals to ALL doctor appointments
- Ensure all documentation is uploaded
- Medication management
- Additional duties assigned per company needs
