Brightwater Care Group is seeking a Support Coordinator to manage a complex client caseload, including Younger Onset Dementia, Huntington’s Disease, acquired brain injury and other neurological conditions. You will be the main contact for clients and families, providing support with package information and up-to-date advocacy details.

Qualifications

• Use a holistic approach to support clients navigate their plan and access appropriate support.
• Sound understanding of current funding streams in the disability sector.
• Experience with the NDIS preferred.
• Ability to complete timely formal reports and client-based issues.

Responsabilités

• Be the main point of contact for clients and families, providing high-level support on package information and advocacy.
• Present opportunities to build partnerships with local organisations and the community.
• Help clients manage their plans to build skills and confidence for independent living.
• Plan and coordinate community-based meetings/visits with clients.
