Qualifications

• Master's degree in social work from an accredited university.
• Current, unencumbered LMSW, LCSW, or LICSW license.
• Must have passed the ASWB Master's-Level exam.
• Minimum 3 years of experience as a social worker in a medical healthcare setting.
• Proficient in Microsoft Word, Outlook, and Excel.

Responsibilities

• Serve as a Licensed Master’s level Social Worker within the SNP program and collaborate with care teams.
• Conduct telephonic outreach to assess social determinants of health and coordinate care using plan benefits.
• Utilize structured assessments to determine interventions like care coordination and resource connection.
• Collaborate with nurses, dietitians, behavioral health, pharmacists, providers, and community partners.
• Handle inbound and outbound member interactions in a telephonic, work-from-home environment and follow up as needed.
• Develop and maintain member care plans to support ongoing care coordination.
• Document all interactions accurately in the member record for CMS compliance.
