Job summary

Humana is seeking a Licensed Master’s Social Worker to join the SNP program in a remote telephonic capacity. You will assess member needs, coordinate care, and collaborate with an interdisciplinary team to improve outcomes.

The role emphasizes structured assessments, care planning, and CMS compliance. You will manage outbound and inbound member interactions, navigate multiple systems, and ensure timely follow-up while maintaining high-quality documentation and adherence to guidelines.

Qualifications

• Master's degree in social work from an accredited university.
• Current, unincumbered LMSW/LCSW/LICSW license.
• Must have passed the ASWB Master's-Level exam.
• Minimum 3 years of experience as a social worker in a medical healthcare setting.
• Proficient in Microsoft Word, Outlook, Excel.
• Capacity to manage multiple priorities with multiple applications.
• Willing to obtain/maintain licensure in multiple states as needed.

Responsibilities

• Serve as a Licensed, Master's-level Social Worker within the SNP program as a Support Social Services team member.
• Conduct telephonic outreach to members with social determinants of health needs to assess and coordinate care.
• Utilize structured assessments to determine appropriate interventions, including care coordination and health education.
• Collaborate with interdisciplinary team members and community partners to address member needs.
• Handle outbound and inbound member interactions in a remote work environment; perform follow-up as needed.
• Assess members for additional support needs and initiate referrals as appropriate.
• Develop and maintain member care plans to support ongoing coordination.
• Document all interactions accurately in the member record to ensure CMS compliance.
