Job summary

Humana is seeking a Licensed Master’s Social Worker to join the SNP care team in a telephonic, work-from-home environment. You will assess members with social determinants of health needs, coordinate care, provide health education, and connect them with community resources.

You will collaborate with interdisciplinary colleagues to address member needs and maintain care plans, ensuring CMS compliance and high-quality care outcomes.

Qualifications

• Must have Master’s degree in social work from an accredited university.
• Current, unincumbered social work license (LMSW, LCSW, or LICSW).
• Must have passed the ASWB Master’s-Level exam.

Responsibilities

• Be a Licensed Master’s Social Worker within SNP on the care team.
• Telephonically outreach to members with social determinants of health needs.
• Use structured assessments to determine interventions and advocacy.
• Collaborate with nurse care managers, dietitians, behavioral health, pharmacists, and providers.
• Manage outbound and inbound member interactions from a home-based setting.
• Initiate referrals and assess for additional support needs.
• Develop and maintain member care plans as needed.
• Document all interactions accurately in the member record for CMS compliance.
