Job summary

UnitedHealth Group’s Optum Care at Home program is seeking a Medical Social Worker to assess psychosocial needs, develop care plans, and coordinate services with an interdisciplinary team in home-based settings across the United States. The role supports patients in staying healthy at home and navigating the care system.

The successful candidate holds an MSW and an unrestricted LCSW/LICSW in NC, with 2+ years of social work experience, and will work remotely within the U.S.

Qualifications

• Master's Degree (MSW) in Social Work.
• Current unrestricted LCSW or LICSW license in the state of North Carolina.
• 2+ years of social work experience (internship not included).

Responsibilities

• Conduct a comprehensive psychosocial assessment of the patients and families.
• Utilize evidence-based screening tools to assess behavioral health conditions.
• Develop person-centered care plan goals with patients, families or caregivers.
• Facilitate serious illness and advance care planning discussions.
• Provide brief therapeutic interventions and solution-focused case management.
• Promote self-management strategies to support empowerment.
• Facilitate referrals for services outside Care at Home as needed.
• Communicate with the interdisciplinary care team to support goals.
• Maintain patient confidentiality and professional standards.
