Qualifications

• Master's Degree (MSW) in Social Work.
• Current unrestricted LCSW or LICSW license in South Carolina.

Responsibilities

• Conduct a comprehensive psychosocial assessment of the patient and family.
• Utilize screening tools to assess behavioral health conditions.
• Develop person-centered care plan goals with patients and caregivers.
• Facilitate serious illness and advance care planning discussions.
• Provide brief therapeutic interventions and case management.
• Promote patient and family self-management and empowerment.
• Coordinate referrals to additional services as needed.
• Maintain patient confidentiality and comply with standards.
