Job summary

All Abilities, Inc. is seeking a full-time Nursing Home Transition Service Coordinator to support participants in transitioning from nursing facilities to home and community in the Hazelton, PA area.

Responsibilities include assisting with housing and utility options, coordinating with nursing home staff and families, checking waiver or funding opportunities, and performing case management across Pennsylvania counties.

Qualifications

• Requires high school diploma or GED.
• Proven ability to communicate and provide excellent customer service.
• Ability to organize multiple tasks in a timely manner.
• Must travel within 50 miles of Hazelton, PA up to 50% per week using a personal vehicle.
• Proficient with Microsoft Office Suite, Outlook and Adobe PDF.
• ACT 33/34 and FBI clearances required; ability to work from home with internet.

Responsibilities

• Assist participants with housing and utility options.
• Coordinate with nursing home staff and participant family members as needed.
• Check on waiver or income funding and related supports.
• May conduct case management across Luzerne, Lackawanna, Carbon, Schuylkill, or Northumberland counties.
