Job summary

UnitedHealth Group is seeking a patient services professional to manage high-touch care for Bleeding Disorders and Alpha1 patients, coordinating monthly refills and ongoing clinical assessments. The role includes onboarding new patients and ensuring continuity of care through a coordinated, team-based approach.

You will liaise with pharmacists, prescribers, nurses, and infusion teams to ensure seamless patient experience, while processing prescriptions and tracking eligibility across systems.

Qualifications

• Experience managing high-touch patient cases in a clinical setting.
• Ability to coordinate care across multiple care teams.
• Proven track record of accurate prescription processing.

Responsibilities

• Manage a dedicated population of Bleeding Disorders and Alpha1 patients through inbound and outbound case management.
• Provide onboarding for new patients, ensuring education and continuity of care.
• Serve as liaison between patients, pharmacists, prescribers, nurses, sales, supply chain and infusion teams.
• Support scheduling and coordination of treatments in ambulatory infusion settings.
• Enter and process prescriptions, verify demographics and insurance, and monitor claims.
