Job summary

Comagine Health is seeking an Improvement Advisor to support implementation and coordination of community health programs in rural Nevada. You will collaborate with project teams, health care providers, and community partners to expand access to evidence-based health promotion and chronic disease programs.

This role involves technical assistance, data collection, and continuous quality improvement, with a focus on strengthening connections across the Southern Nevada Pathways Community HUB.

Qualifications

• Bachelor’s degree in public health, health care, community health, or related field.
• 5 years of health care, public health, community-based services, or related experience with quality improvement or program implementation.
• Experience with health care providers, community-based organizations, or community health programs.
• Strong knowledge of quality improvement methods and care coordination.
• Excellent communication, facilitation, and partner engagement skills.
• Ability to work with rural Nevada communities.

Responsibilities

• Guide health care and community organizations through project planning and quality improvement activities.
• Provide technical assistance and implementation support to rural partners.
• Support development and improvement of referral and enrollment workflows.
• Strengthen recruitment, enrollment, retention, and access to programs.
• Support community outreach strategies and collaboration with local organizations.
• Coach partners on data collection, interpretation, and performance monitoring.
• Facilitate trainings, partner meetings, and learning opportunities.
• Monitor progress and address barriers to program delivery.
• Document activities using CRM and project management tools.
