Job summary

Qualifications

• Current, unrestricted RN license in the State of Wisconsin.
• 2+ years of hospital, acute care, direct care or home health case management experience.
• Basic level of experience with Microsoft Office and EMR applications.

Responsibilities

• Making outbound calls to assess members' health status.
• Conducting face-to-face assessments in the member's home.
• Identifying gaps or barriers in treatment plans.
• Coordinating care and making referrals to outside sources.
• Educating members on disease processes and self-management.
