Qualifications

• Current, unrestricted RN license in Wisconsin.
• 2+ years of experience in hospital/acute care or Home Health Case Management.
• Reliable transportation to travel to member homes within the geographical area.
• Experience with electronic medical records (EMR).

Responsibilities

• Make outbound calls to assess members' current health status.
• Conduct face-to-face assessments in the member's home.
• Assist members in identifying unmet needs and coordinating services.
• Document assessment details and apply State guidelines to requests for services.
• Document and track findings to inform care planning.
• Coordinate care and referrals to outside sources as needed.
