Responsibilities

• Make outbound calls to assess members' health status.
• Conduct face-to-face home assessments.
• Assist members in identifying unmet needs and apply state guidelines.
• Document assessment details and track findings.
• Identify gaps or barriers in treatment plans.
• Provide patient education for self-management.
• Interact with Medical Directors on challenging cases.
• Coordinate care and make referrals to outside sources.
• Arrange services (home health, DME) as needed.
• Educate members on disease processes and healthy lifestyle changes.
