Job summary

UnitedHealth Group in Milwaukee, WI is hiring a Utilization Case Manager to serve Milwaukee County with occasional travel to surrounding counties. Qualifications

• Current, unrestricted RN license in Wisconsin.
• 2+ years of hospital/acute care or Home Health Case Manager experience.
• Reliable transportation to travel to member homes within area and up to 25% travel.

Responsibilities

• Make outbound calls to assess members' health status.
• Conduct face-to-face assessments in the member's home.
• Identify unmet needs and coordinate services (home health, DME, etc.).
• Document assessments and apply state guidelines to service requests.
• Educate members on disease processes and healthy lifestyle changes.
• Coordinate care with Medical Directors on challenging cases.
