Job summary

Call 4 Health is seeking a Registered Nurse (RN) to support a remote, multi-state nurse triage service as a physician extender. The RN will manage EHRs, respond to patient inquiries, and provide timely clinical guidance.

Triage calls, document interactions, coordinate follow-up care and referrals, and educate patients on chronic disease management. Requirements: active eNLC license and 2–4 years of RN experience; experience with at least two EHR platforms. This position is remote.

Qualifications

Associate’s or Bachelor’s Degree in Nursing.
2–4 years of full‑time clinical RN experience.
Active eNLC license required.
Experience with at least two different EHR platforms.
Telephone triage experience using electronic triage systems.

Responsibilities

Track and respond to calls for a multi-state nurse triage service using established protocols.
Provide clinical assessments and triage patients by telephone or portals.
Perform admin and clinical support tasks including medication refills and follow-up coordination.
Facilitate referrals and assist with prior authorization requests and inquiries.
Participate in RPM programs and educate patients on chronic disease management.
Screen for clinical trials and conduct follow-up outreach as needed.
