Job summary

Anchor Health is seeking a compassionate RN Triage Nurse (Full-Time) to join a supportive team providing hospice/palliative care in Petaluma, CA. The Triage Nurse handles nurse line triage, coordinates with clinics, and ensures quality patient care in compliance with regulations.

Requirements include CA RN license and BLS, with at least 1 year in ambulatory care. Excellent communication and computer skills are essential; NextGen EMR experience is a plus.

Qualifications

• California RN license required.
• BLS certification from AHA or ARC required.
• Ambulatory care experience preferred.
• Strong communication and organizational skills required.
• Proficient with computers/word processing.

Responsibilities

• Responds to triage calls and provides guidance.
• Manages pharmacy line inquiries and responses.
• Performs telephone-based patient assessments.
• Determines urgency using medical history in EMR.
• Escalates high-risk cases to ER or ambulance coordination.
• Documents consultations in NextGen EMR.
• Coordinates non-emergency appointments with physicians.
