Job summary

Call 4 Health is seeking experienced Registered Nurses to join our 24-hour remote triage center, delivering triage assessments, documentation, and clinical support across multiple states. You will work from home with a HIPAA-compliant setup, using Schmitt-Thompson protocols, and collaborate with providers, hospitals, and patient care teams while maintaining flexibility and patient-centered care.

This role requires active RN licensure, 2–4 years of RN experience, and willingness to obtain

Qualifications

• RN licensure active and unencumbered
• 2–4 years of full-time RN clinical experience (ER/Urgent Care/Adult/Pediatric/Ambulatory/ICU)
• Experience with two or more EHR platforms
• Willingness to obtain additional state licenses

Responsibilities

• Respond to inbound/outbound calls for a multi-state nurse triage service
• Apply Schmitt-Thompson protocols to assess and triage patients
• Document patient interactions in client EHR systems
• Provide clinical assessments via phone or patient portal
• Support providers with administrative and clinical tasks
• Coordinate referrals and prior authorization with clinical input
• Collaborate with hospitals, nursing homes, and other agencies as needed
• Participate in Remote Patient Monitoring (RPM) including vital signs monitoring
• Screen and qualify patients for clinical trials
• Take on additional responsibilities to support team and patient outcomes
