Job summary

SCAN Group is seeking a Registered Nurse (RN) – Care Management to deliver high-touch, person-centered care for members with complex medical needs. The role combines in-home and telephonic assessments, care plan development, and education for patients and caregivers.

You will coordinate with a multidisciplinary team, support transitions of care, and ensure timely documentation across systems. Travel 50% in Los Angeles and surrounding areas is expected.

Qualifications

• ASN required.
• CA RN license required.
• CCM certification strongly preferred.
• Travel 50% of the time; valid driver's license and reliable transportation.
• Bilingual English/Spanish required; test may be administered.

Responsibilities

• Conduct in-home and telephonic assessments for high-risk members.
• Develop and manage individualized care plans with measurable goals.
• Educate members and caregivers on disease processes and self-management.
• Coordinate with interdisciplinary care team to improve outcomes.
• Ensure documentation across computer systems within required timeframes.
