Job summary

Vaya Health seeks a Registered Nurse to perform needs-based Medicaid PCS assessments for beneficiaries in home or community settings. You will schedule visits, conduct telephonic and in-person assessments, and coordinate with prescribers to ensure complete SAR submissions.

The role requires NC RN licensure, a bachelor’s degree, and the ability to travel within Alamance, Chatham and Caswell counties. This is a remote role with NC residency requirements.

Qualifications

• Bachelor’s degree in Nursing or healthcare field.
• Active RN licensure in North Carolina.
• Experience in home health or community-based care preferred.

Responsibilities

• Schedule assessments with members, families and providers.
• Conduct needs-based PCS eligibility assessments.
• Submit SARs on behalf of PCS providers.
• Maintain privacy and dignity of beneficiaries during assessments.
• Travel to counties as required and participate in meetings.
