Job summary

Vaya Health is seeking a Registered Nurse to perform needs-based Medicaid PCS assessments in western North Carolina, serving multiple counties. The role involves telephonic and home visits, coordinating with prescribers and PCS providers, and submitting service authorization requests.

Travel to counties as needed is required. The ideal candidate holds a Bachelor’s degree in Nursing, active NC RN licensure, and at least two years of home care experience, with PCS assessment knowledge preferred.

Qualifications

• Bachelor’s degree in Nursing and NC RN license are required.
• Two years of experience in home care is preferred.
• Experience conducting PCS assessments is highly preferred.

Responsibilities

• Schedule assessments with members, families and providers.
• Determine PCS eligibility using standardized tools.
• Conduct interviews with family members and caregivers.
• Submit SARs for PCS providers and maintain privacy standards.
• Participate in training and stakeholder education as needed.
