Allegheny Health Network is seeking an RN Nurse Navigator for a hybrid role based in Pennsylvania. You will serve as the main contact for referring physicians, patients and families, coordinating care and guiding access to clinical and supportive services across the network.

Responsibilities include coordinating appointments, educating patients about treatment options, and collaborating with multidisciplinary teams.

Formación

• Current PA RN license or multi-state eNLC license.
• 1–3 years of nursing experience in the specialty.
• Bachelor's degree in Nursing preferred.
• Certification in area of expertise preferred.
• CPR certification (American Heart Association).
• Background clearances as required by law.

Responsabilidades

• Acts as primary contact for referring physicians, patients, families, and community.
• Coordinates patient care and access to clinical and supportive services.
• Educates patients on diagnosis, treatment options, and care plans.
• Coordinates appointments with multidisciplinary teams and supports continuity of care.
• Identifies gaps in care and facilitates community outreach and education.
