Job summary

Qualifications

• RN with at least 2 years of relevant specialty nursing experience (e.g., oncology, surgery, orthopedics).
• BSN preferred; RN licensure required.
• Nurse Navigation training or related certification preferred.
• Strong communication, leadership and problem-solving abilities.
• Ability to work independently and within a multidisciplinary team.

Responsibilities

• Reviews patient baseline data across medical, psychosocial and financial domains.
• Assesses educational needs of patient and family related to disease and treatment.
• Collaborates with the care team to create/update the plan of care.
• Provides education on resources and helps set short and long-term goals.
• Coordinates referrals and communicates plan of care across departments.
• Maintains EMR documentation and supports continuity of care.
• Participates in multidisciplinary conferences and education programs.
