Qualifications

• 4+ years experience treating CHF, hypertension, and Diabetes in outpatient or inpatient settings.
• Compact multi‑state RN licensure (RN compact).
• Experience with remote patient monitoring technology.

Responsibilities

• Monitor vitals, symptoms, and labs to identify patients needing clinical interventions.
• Follow up with patients with abnormal readings and triage appropriately.
• Respond to inbound clinical questions and escalations by phone/text.
• Lead virtual follow‑ups to support enrollment, treatment plan changes, and medication adherence.
• Support documentation and care summaries for patients' physicians.
• Ensure every interaction prioritizes patient well‑being and aligns with health system goals.
• Contribute to scaling care delivery for chronic conditions like CHF, hypertension, and diabetes.
