Acentra Health is seeking a Clinical Assessor to join our team in North Carolina. You will perform needs-based assessments for CAP and PCS beneficiaries, typically in the beneficiary’s home or in community settings.

The role is hybrid and requires NC licensure (RN or LCSW) with at least two years of nursing experience, and the ability to travel within a 60‑mile radius to assessment locations across NC.

Qualifications

• RN or LCSW license in NC
• Minimum of two years nursing experience
• Willingness to travel up to 60 miles radius
• Hybrid work arrangement in NC (Anson/Union counties)

Responsibilities

• Perform needs-based LOC assessments for CAP/PCS beneficiaries in home/community settings.
• Complete eligibility determinations for Medicaid-funded personal care services.
• Coordinate with stakeholders and document findings to support care decisions.
