Job summary

Acentra Health is seeking a Clinical Assessor to join our team in North Carolina. The role involves hybrid work, performing needs-based assessments for PCS and CAP programs, and coordinating with families and case management to ensure appropriate services. Travel within the region is required.

The assessor will conduct initial eligibility determinations under HCBS waivers, develop care plans, and participate in mediation and appeal processes while adhering to HIPAA and policy guidelines.

Qualifications

• Registered Nurse license (NC or Compact) required.
• Minimum two years nursing experience.
• Willingness to travel up to 60 miles.
• Ability to work hybrid/home-based assessments.

Responsibilities

• Conducts needs-based assessments to determine PCS eligibility.
• Ensures PCS are provided on a needs basis per unmet needs.
• Includes interview with family and informal caregivers.
• Provides guidance to select PCS providers and conducts service plan reviews.
• Participates in mediation, appeals, and state inquiries.
