Vitability Health of New Jersey is seeking a compassionate Chronic Care Manager (RN) to remotely manage a caseload of patients with multiple chronic conditions. You will build personalized care plans, conduct comprehensive assessments, and guide patients through disease management using telehealth and remote monitoring.

The role emphasizes patient-centered care, collaboration with care navigators, and ongoing documentation within HIPAA-compliant EHRs to support better outcomes.

Qualifikationen

• Active RN license in New Jersey or a valid Compact RN license
• Experience in Chronic Care Management (CCM)
• Required acute care nursing experience
• Ability to work remotely and independently

• Provide ongoing chronic care management to a caseload of patients with multiple chronic conditions
• Develop and maintain personalized care plans reflecting each patient’s goals and needs
• Conduct comprehensive assessments of medical, social, and lifestyle factors affecting health
• Monitor patient progress, identify changes, and adjust care plans to improve outcomes
• Educate patients and families on disease management, medications, and healthy lifestyle choices
• Use telehealth and remote monitoring to stay connected with patients between visits
• Collaborate with Care Navigators and the care team to ensure high-quality care
• Coordinate appointments, tests, and follow-ups to reduce barriers to care
• Assist with medication reconciliation and understanding treatment plans
• Track patient outcomes and quality measures to strengthen care programs
• Maintain compliant documentation in the EHR per HIPAA and CMS guidelines
