Job summary

Qualifications

• Active RN license is required.
• Experience with chronic or medically complex patients is preferred.
• Strong clinical judgment and ability to educate patients.
• Comfortable with technology and virtual patient care.
• Ability to work independently in a fast-paced remote environment.
• Experience with telehealth or remote patient management is a plus.
• Background in ER/ICU/Stepdown or related areas is relevant.
• Bilingual Spanish is a plus.

Responsibilities

• Onboard new patients and families in a virtual setting.
• Develop individualized care plans for chronic conditions.
• Provide ongoing follow-up and monthly check-ins.
• Assess changes and escalate concerns as needed.
• Educate patients and caregivers on care needs.
• Coordinate referrals, medications, appointments, and resources.
• Collaborate with clinical and technology teams to evolve workflows.
