Qualifications

• RN with 5+ years of experience, including hospital setting.
• Active multistate license via NLC or state-specific licenses for each state supported.
• 1+ year of experience documenting electronically.
• 1+ year of Oncology experience.

Responsibilities

• Perform telephonic case management with patients and care teams.
• Apply criteria, guidelines, policies, and regulatory standards to determine benefits and care plans.
• Use clinical judgment to reduce risk factors and address barriers in care planning.
• Assess multiple conditions and comorbidities affecting functionality.
• Collaborate with supervisors and multidisciplinary teams to meet goals.
