CVS Health is seeking a remote RN Case Manager to provide telephonic case management for members across multiple states. You will assess care needs, apply guidelines, and coordinate with care teams to optimize outcomes. A multi-state license is required and experience across hospital and specialty areas is valued.

Qualifications

• Active RN license with multi-state license per NLC.
• 5+ years of RN experience, including hospital setting.
• Experience documenting electronically using a keyboard.
• Experience in Oncology, Transplant, Specialty Pharmacy, Pediatrics, Medical/Surgical, Behavioral Health/Substance Abuse or Maternity/Obstetrics.

Responsibilities

• Provide telephonic case management for fully and/or self-insured clients.
• Apply clinical guidelines and policies to determine benefits and care plans.
• Assess and address barriers to care and risk factors.
• Collaborate with supervisors and multidisciplinary teams for case conferences.
• Use motivational interviewing to engage members and assess health needs.
• Perform medical necessity reviews.
