Job summary

CVS Health is seeking a remote RN Case Manager to support members across multiple states from home in the US. You will conduct telephonic case management, apply clinical guidelines, coordinate care with providers, and advocate for cost-effective quality care for fully and self-insured clients.

The role requires RN licensure (multistate preferred), 5+ years of RN experience including hospital settings, and oncology exposure.

Qualifications

• 5+ years' experience as a Registered Nurse.
• Active multistate license through the Nurse Licensure Compact (NLC).
• 1+ years' experience documenting electronically.
• 1+ years' experience in Oncology.

Responsibilities

• Telephonic case management with patients and care teams for fully/self-insured clients.
• Apply criteria and clinical guidelines to assess benefits and member needs.
• Use clinical judgment to address barriers and plan care to reduce risk factors.
• Assess conditions from multiple sources, including co-morbidities.
• Consult with supervisors and teams to meet goals and objectives.
• Engage members using motivational interviewing to determine health needs.
