Job summary

CVS Health is seeking a remote RN Case Manager to support members across multiple states through telephonic care management. You will assess needs, coordinate with care teams, and ensure appropriate benefits and care plans.

Requirements include 5+ years as an RN, a multistate license via the NLC, oncology experience, and electronic health record documentation. Bilingual Spanish is preferred. Comprehensive benefits are included for full-time employees.

Qualifications

• RN with 5+ years of nursing experience in hospital settings.
• Active multistate license via Nurse Licensure Compact or state-specific licenses for non-compact states.
• 1+ years experience documenting electronically (EHR).
• 1+ years Oncology experience.
• CCM URAC accreditation preferred.

Responsibilities

• Work telephonically with patients and care teams for managed care clients.
• Apply criteria, clinical guidelines, and care management plans to assess benefits and needs.
• Identify barriers, engage members, and guide care to meet goals.
• Collaborate with supervisors and medical directors; present cases as needed.
• Perform medical necessity reviews and coordinate transitions to lower levels of care.
