Job summary

CVS Health is seeking a Community Care Case Manager (RN) to coordinate integrated care for members and families. You will assess needs, plan care, advocate for resources, and partner with providers, insurers, and employers to promote quality outcomes.

You will communicate with patients and care teams, document activities, and support return-to-work goals while adhering to HIPAA and regulatory requirements.

Qualifications

• Active and unrestricted Compact RN licensure in OH or state of residence.
• 3+ years clinical experience in diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac with Medicare members.
• 2+ years case management, discharge planning and/or home health care coordination experience.

Responsibilities

• Act as liaison with member/family, employer, providers, and insurers.
• Coordinate case management activities across the continuum of care.
• Interact with members telephonically or in person; may visit homes, worksites, or physician offices.
• Assess medical/vocational status and develop a care plan to optimize wellness and outcomes.
• Prepare documentation of case work and communicate with stakeholders.
• Ensure compliance with laws, regulations, and provider requirements.
