Job summary

Elevance Health is seeking a Telephonic Nurse Case Manager II (AmeriBen) to support members with complex and chronic care needs. This role is primarily telephonic, with required in-person trainings and a hybrid/virtual work approach.

The candidate should have a BA/BS in a health-related field and an active RN license, with multi-state licensure as needed. You will develop, coordinate, and evaluate care plans while working with Medical Directors and providers to optimize outcomes.

Qualifications

• BA/BS in a health-related field required or equivalent background.
• Current, unrestricted RN license; multi-state licensure required if practicing across states.
• Experience with utilization review, prior authorization, and care management preferred.

Responsibilities

• Assess, develop, implement, coordinate, monitor and evaluate care plans telephonically or on-site.
• Ensure access to services and coordinate resources to meet member needs.
• Collaborate with Medical Directors and providers on care plans and treatment decisions.
• Monitor effectiveness and modify care plans as needed.
