Elevance Health is seeking a Telephonic Nurse Case Manager II to coordinate care for members with complex needs. The role emphasizes telephonic care management across the care continuum and collaboration with clinicians and care teams.

Requires an active RN license, a BA/BS in a health-related field, and 5+ years of clinical experience. This is a hybrid position with some in-person onboarding and training sessions, and a multi-state licensure is often required.

Qualifications

• Requires BA/BS in a health-related field and a minimum of 5 years of clinical experience; or an equivalent background.
• Current, unrestricted RN license in applicable state(s) is required; multi-state licensure if serving multiple states.

Responsabilités

• Provide care management telephonically for members with complex and chronic needs.
• Assess, develop, implement, coordinate, monitor and evaluate care plans.
• Facilitate authorizations/referrals within benefits structure or via extra-contractual arrangements.
• Coordinate internal and external resources to meet identified needs.
• Monitor and adjust care plans; collaborate with Medical Directors and Physician Advisors.
