Job summary

Lancesoft is seeking a Nurse Case Manager II for a fully remote Illinois-based role. The position focuses on telephonic and face-to-face assessments, care planning, and coordination to meet medical and behavioral health needs across the continuum of care.

Responsibilities include comprehensive assessments, crisis intervention, and collaboration with the care team to ensure optimal, cost-effective outcomes. Licensure in IL is required.

Qualifications

• Minimum Associate's degree or diploma in nursing.
• 3-5 years clinical practice experience (hospital or ambulatory care).
• 2+ years using a computer and MS Office Suite (Teams, Outlook, Word, Excel).
• BSN preferred.
• Case management in an integrated model preferred.
• Managed care experience preferred.
• Discharge planning experience preferred.
• Experience providing care to Medicaid population preferred.
• Spanish Speaking Preferred.
• RN license with current unrestricted IL state licensure required.

Responsibilities

• Assess, plan, implement and coordinate care management activities to meet members' medical and behavioral health needs.
• Develop proactive action plans to address issues and enhance long-term outcomes.
• Conduct comprehensive assessments using clinical tools and data review.
• Apply clinical judgment to reduce risk factors and address complex health indicators.
• Coordinate transitions of care and ensure appropriate administration of benefits.
• Provide crisis intervention and follow-up for behavioral health or medical crises.
• Serve as a single point of contact to remediate gaps in care and access.
• Collaborate with multidisciplinary team to overcome barriers to goals and objectives.
• Present cases at conferences for multidisciplinary input.
• Utilize care management processes in compliance with policies and procedures.
