Job summary

CVS Health in Illinois is seeking a Community Care Case Manager (RN) to assess, plan, coordinate, and advocate for members' health needs with the aim of quality outcomes and timely return to work.

You will collaborate with multidisciplinary teams, ensure regulatory compliance, and may travel within a designated area. Requires IL RN license and 3+ years clinical experience; remote work option provided with strong documentation skills.

Qualifications

• Active and unrestricted IL RN license.
• 3+ years clinical practice; preference: diabetes, CHF, CKD, post-acute care, hospice, palliative care, Medicare.
• 2+ years case management, discharge planning, or home health coordination.

Responsibilities

• Act as liaison with member/client, family, employer, providers, insurers, and healthcare personnel.
• Coordinate all case management activities for acute, chronic, or catastrophic cases across the care continuum.
• Interact with members/clients telephonically or in person; may visit homes, worksites, or physician offices.
• Assess medical and vocational status; develop a plan of care to optimize wellness and return to work.
• Communicate with stakeholders and document case work activities.
• Collaborate with internal multidisciplinary teams to maximize health outcomes.
• Outreach to treating physicians or specialists when appropriate.
• Provide educational and preventive information for better outcomes.
• Ensure compliance with laws and insurance carrier requirements.
