Responsibilities

• Uses clinical tools and information/data to evaluate member needs and benefits.
• Applies clinical judgment to reduce risk factors and address barriers and indicators affecting care planning.
• Conducts assessments using information from multiple sources (e.g., claims) to address conditions impacting function.
• Assesses need for referrals to clinical resources and interdisciplinary team members.
• Collaborates with supervisor and stakeholders; presents cases at interdisciplinary conferences.
• Follows regulatory and company policies; uses motivational interviewing to engage members.
