Responsibilities

• Provide telephonic engagement with members using motivational interviewing.
• Develop and monitor individualized care plans with follow-up actions and goals.
• Coordinate referrals and connect members to providers and services.
• Prepare for and participate in audits, rounds, and required training; mentor new hires.
