Job summary

Qualifications

• Minimum three years of experience in Care Management, Primary Care, or home care.
• Experience working with older adults and families preferred.

Responsibilities

• Perform comprehensive assessments and develop complex plans of care.
• Coordinate medical appointments, medication refills, therapy sessions, and transportation.
• Conduct follow-up calls and periodic in-home visits to evaluate services.
• Collaborate with physicians, pharmacists, homecare agencies, and multidisciplinary teams.
