Job summary

Baptist Home Health Care by BAYADA seeks an experienced RN to serve as Transitional Care Manager in the Jacksonville, FL area. The TCM will coordinate initiation of home health services, build relationships with discharge planners and case managers, and educate patients about care eligibility and BAYADA services.

The role involves managing referrals, liaising with physicians and health care teams, and ensuring smooth transitions from hospital to home care.

Qualifications

• RN license in good standing in Florida.
• Graduate of an accredited nursing program.
• Strong interpersonal and communication skills.

Responsibilities

• Develop relationships with referral sources, physicians and other professionals.
• Market and clearly articulate BAYADA services and benefits.
• Demonstrate mastery of home care eligibility criteria.
• Manage hospital transitions of care and educate patients about BAYADA services.
• Review medical documentation and assess eligibility for home care.
• Complete weekly referral log.
