Responsibilities

• Evaluates member needs and barriers to care and recommends a plan for outcomes.
• Develops ongoing care/service plans and collaborates with providers to identify resources.
• Identifies problems and provides appropriate care management interventions.
• Coordinates between member/family and care team to ensure person-centered care.
• Provides follow up and monitors status and revises care plans as needed.
• Offers resource support for local services based on assessments.
• Facilitates care management to ensure timely access to care.
• Performs outreach via telephonic, digital, or home visits when needed.
• Collects and maintains member information to comply with regulators.
• Delivers education to members and families on disease processes and benefits.
• Provides feedback to leadership to improve care delivery and cost effectiveness.
• Performs other duties as assigned and compliant with policies.
