Job summary

Centene is seeking a Care Manager (RN) to lead complex care management for Medicaid members in a remote role serving Georgia residents. You will assess needs, develop care plans, and collaborate with cross-functional teams to ensure timely access to services.

The role requires an RN license, a BSN or accredited nursing degree, and 2–4 years of related experience. Flexible schedules and remote work are available for eligible candidates.

Qualifications

• RN licensure in state and/or compact
• 2–4 years of related experience in care management or nursing
• Degree from an accredited nursing program or BSN required

Responsibilities

• Evaluate member needs and barriers to accessing appropriate care.
• Develop ongoing care plans and collaborate with providers.
• Coordinate between members, families, and care teams to ensure timely access to services.
• Provide ongoing follow-up, monitoring, and documentation for compliance.
• Educate members and families on disease processes and care options.
