Job summary

SCAN Health Plan is seeking a licensed RN to support frail seniors remotely in a part-time telephonic role during Pacific Time business hours. The RN will conduct assessments, design and monitor care plans, and coach members to manage chronic conditions and navigate complex care systems.

Requirements include CA RN licensure and ADN/ASN; BSN preferred. Prior managed care or LTSS experience and bilingual English/Spanish preferred.

Qualifications

• CA RN license required.
• ADN/ASN required; BSN preferred.
• Experience in managed care or case management.

Responsibilities

• Perform initial and annual telephonic assessments for Medi-Cal/Medicare dually eligible members.
• Develop and implement plans of care with the care team; document in multiple systems.
• Provide health education and coaching to manage chronic conditions and prevent ER visits.
