Job summary

Molina Healthcare, Inc. is seeking a Care Manager RN to support care coordination for LTSS, including home visits and waiver services. You will collaborate with an interdisciplinary team to ensure high-need members progress toward outcomes while maintaining cost-effective, quality care.

You will assess medical necessity, coordinate with caregivers and providers, and navigate waiver processes. Significant local travel is required, with remote coordination as needed.

Qualifications

• Active RN license in state of practice.
• 2+ years in health care with 1+ year in care management or LTSS.
• Valid driver's license and reliable transportation.
• Experience with disabilities, chronic conditions or substance use disorders preferred.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits.
• Facilitate waiver enrollment and disenrollment processes.
• Develop and implement care plans with members, caregivers, physicians and care network.
• Monitor care plan effectiveness and document interventions and outcomes.
• Coordinate LTSS, behavioral health, and community resources for continuity of care.
• Assess medical necessity and authorize waiver services.
• Lead ICT meetings for service approvals and collaborations.
• Travel 25-40% locally as required by state/contractuals.
• Provide consultation and support to peers as needed.
