Job summary

Molina Healthcare, Inc. is seeking an RN Care Manager for a remote, field-based role with travel across McKinley County, NM.

You will support care management activities, collaborate with a multidisciplinary team, and coordinate integrated member care to improve outcomes and manage costs. Responsibilities include comprehensive assessments, developing care plans, conducting visits, monitoring progress, and promoting service integration across behavioral health and LTSS.

Qualifications

• At least 2 years experience in health care, preferably in care management.
• RN license must be active and unrestricted in state of practice.
• Valid and unrestricted driver’s license and reliable transportation.
• Understanding of EMR and HIPAA compliance.
• Ability to work independently with minimal supervision and multitask.
• Strong communication, time management and problem-solving skills.
• Knowledge of community resources and care coordination.

Responsibilities

• Completes comprehensive assessments per timelines and identifies care management eligibility.
• Develops and implements care coordination plans with the care team and network.
• Conducts telephonic, in-person or home visits as required.
• Monitors progress, documents interventions and updates plans as needed.
• Maintains ongoing member caseload and outreach for management.
• Promotes integration of behavioral health, LTSS, and community resources.
• Facilitates ICT meetings and collaboration with the team.
• Uses motivational interviewing to educate and motivate change.
• Assesses barriers to care and provides coordination support.
• May provide consultation to peers as needed.
• Travel will be required (roughly 25–40%).
