Job summary

VillageCare in the New York Tri-State area is seeking a Full Time RN- Care Manager. This hybrid role requires NYS RN license, 3+ years in managed care, home care, or community settings, and bilingual capabilities.

Qualifications

• Valid NYS RN license with 3+ years in managed care, home care, or community settings.
• Associate's degree or higher required.
• Strong communication and problem-solving abilities.
• Bilingual proficiency in Korean, Bengali, Cantonese, Mandarin, Spanish, Russian, or Creole.

Responsibilities

• Care planning, coordination, and health assessments for patients.
• Monitor service quality and progress toward patient goals.
• Oversee hospital or nursing home transitions and discharge planning.
• Identify health, environmental, and psychosocial risks and implement interventions with the interdisciplinary team.
