Job summary

Alpine is hiring a Director, Clinical Operations to oversee care coordination, navigation and management for Colorado Medicaid members under a regional contract. You will lead the Care Coordination team, drive program design, ensure compliance with state requirements, and collaborate with health plan partners and providers.

This role may require travel within the eight-county service area and occasional evenings.

Qualifications

• Master’s degree in Nursing, Healthcare Administration, Public Health, or related field.
• Active unrestricted RN license in Colorado.

Responsibilities

• Provides leadership on care navigation, coordination and management program design.
• Serves as Director of Care Coordination for the state contract.
• Uses outcomes data to inform care and condition management programs.
• Develops proactive policies to prevent deterioration of member conditions.
• Oversees delivery of care coordination for members by network providers and partners.
• Partners with clinical leaders to align with ACC Phase III quality measures.
