Job summary

Optum is seeking an RN Care Coordinator to join the Insight ACO Practice Extend Team in Arizona. You will work with a Clinical Pharmacist and Administrative Coordinator to improve health outcomes, focusing on care transitions and high-risk patient management.

Role involves high-volume telephonic outreach, patient education, and coordination with PCPs and specialists to ensure quality care delivery. Telecommuting within Arizona is available under policy.

Qualifications

• Bachelor’s degree required.
• Current, active, unrestricted RN license in a Compact state within the US.
• Minimum 3 years in Case Management or Care Coordination.
• Proficiency in Microsoft Office (Excel, PowerPoint).
• Ability to work one late shift per week (10am–7pm, AZ time).

Responsibilities

• Support patient and provider engagement across care transitions and medication management.
• Communicate discharge information to primary care offices using appropriate tools.
• Conduct post-discharge outreach, review medications, and schedule follow-ups.
• Guide patients to appropriate and cost-effective specialist referrals.
• Coordinate high-risk patients' PCP or specialist appointments.
• Provide patients with benefit information and resources.
• Collaborate with practice staff and care team to resolve issues.
